Submit this form to apply for a new participant. Bus passes will only be approved if ALL eligibility criteria are met. To ensure that your request is processed quickly, please do not submit a request until all documentation is in order. MHID staff will reach out to you with any follow-up questions after determining the eligibility of the request. |

Use this form to track services accessed through the Connector Card program.

Connector Card Annual Review |
This form is used to determine whether a client is eligible to receive a three-month extension on their active Connector Card after one year. This is required for all clients who still have an active Connector Card at the one-year mark. Please read all instructions on the form before completing it. If a client has been Permanently Housed, please complete the client's CE Exit to Permanent Housing before completing the Annual Review form.

About the Program
The Connector Card program is a one-year bus pass program for chronically homeless clients in partnership with WeGo.
- In order to be eligible, your client must:
- Be 18+ years old
- Be literally homeless, as defined by HUD
- OR Individual:
- Is exiting an institution (e.g., jail, hospital) where (s)he resided for 90 days or less; and
- Resided in emergency shelter or place not meant for human habitation immediately before entering that institution
- Individual is currently staying in:
- Place not meant for human habitation (e.g., car, streets, encampment);
- Emergency shelter;
- Transitional housing;
- OR Individual:
- Have experienced 5+ months of continuous homelessness (round up)
- Have a disabling condition
- Have a CE entry in HMIS
- Have required documentation uploaded to HMIS
- Be willing to work with Housing Navigator
