If you are working with someone that is wanted a Covid 19 vaccination, fill out the above form and return to UNHS. Vaccinations are also available on a walk-in basis at their Downtown Clinic location on Friday mornings.
Mainstream Resources Checklist
The Mainstream Resources Checklist is available to all navigators and should be used as a guide to ensure that all people in a housing crisis are being connected to mainstream services. Not everyone experiencing homelessness will be referred to housing through CE, but that does not mean they shouldn't be connected to other resources!

Coordinated Entry Polices and Procedures Updated July 2020
On July 19th, the CoC General Membership voted to approve the Policies and Procedures Manual, which provides information about how our Coordinated Entry process currently functions in Nashville. The Manual is subject to revision as our community improves its coordinated entry processes.

The cheat sheet includes general information about CE, as well as information on the VI-SPDAT and CE Entries, Updates, and Exits in HMIS. This is a living document, and we may make updates to the cheat sheet. We will keep this site updated with the most recent version of the cheat sheet.
